Abstract

Le cas rapporté est celui d’un patient de 39 ans avec une fracture comminutive de la tête radiale et une luxation radioulnaire distale homolatérale évoquant un syndrome d’EssexLopresti Un traitement orthopédique et 7 mois de rééducation avaient abouti à un échec L’examen clinique retrouvait un angle de flexion du coude de 20 à 140° une supination de 0° une pronation à 40° une flexion de poignet de 40° contre 45° en extension La prise de force au Jamar® était de 18 kg contre 40 kg du côté controlatéral Le bilan radiologique retrouvait une ascension du radius avec un conflit ulnocarpien douloureux Le traitement chirurgical était fait à 9 mois de l’accident avec une résection et secondairement arthroplastie de la tête radiale un accourcissement de l’ulna de 15 mm et une ligamentoplastie utilisant un tendon semitendineux Au 2ième mois le patient reprenait son activité professionnelle Le recul maximal était de 13 mois Les mobilités du coude étaient de 30–130° en flexionextension avec une supination à 45° et une pronation à 60° La prise de force était de 40 kg contre 50 kg en controlatéral Le patient était indolore au repos et dans les gestes de la vie quotidienne Notre technique de ligamentoplastie de la membrane interosseuse est originale par le type et le trajet du transplant utilisé Elle est proposée en complément du traitement chirurgical des fractures de la tête radiale associées à une instabilité longitudinale de l’avantbras Elle éviterait ainsi une surcharge de l’ostéosynthèse ou de la prothèse de tête radialeThe case of a 39yearold patient with an EssexLopresti syndrome is reported The radial head fracture was comminuted and the distal radioulnar joint was dislocated The patient underwent an orthopaedic treatment and a 7month long rehabilitation Motions were assessed with 20–140° elbow flexion 0° supination 40° pronation 40° wrist flexion and 45° extension wrist Jamar® measurement was 18 kg 40 kg for the opposite limb Xrays displayed a radial ascension with an ulnocarpal impingement syndrome Surgical procedures were a shortening of the ulna a radial head arthroplasty and a ligamentoplasty using the semitendinous muscle Two months after surgery the patient worked again The last followup was 13 months after surgery Motions were assessed with 30–130° elbow flexion 45° supination and 60° pronation Jamar® measurement was 40 kg 50 kg for the opposite limb with a painless forearm or wrist Our technique of ligamentoplasty is original with the type and the course of the transplant Its indications would be in radial head lesions associated with a longitudinal forearm instability The ligamentoplasty would prevent from overloads of radial head osteosyntheses or arthroplasties
