Abstract
Introduction Patients with either overt or subliclinical hypothyroidism exhibit increased susceptibility to develop atherosclerotic disease Since the proinflammatory activation of vascular endothelial cells and platelets represents a crucial step in atherogenesis we evaluated the effect of recombinant human thyrotropin rhTSH on biomarkers of vascular endothelial cell and platelet activation in patients monitored for thyroid carcinoma remnantMethods Circulating levels of solubles intercellular adhesion molecule ICA M1 and sEselectin as indices of vascular endothelial cell activation of sPselectin and sCD40 ligand sCD40L as indices of platelet activation and of 8isoprostaglandinPGF2alfa as index of lipid peroxidation were evaluated in 20 patients 16 females 480±136 years at baseline and after intramuscular rhTSH injection 09 mg/day on 2 consecutive daysResults At baseline serum thyrotropin values were below while free T3 and free T4 within the normal range After rhTSH injection serum thyrotropin significantly increased while free T3 and free T4 remained unchanged Concomitantly plasma sICAM1 from 155 9±391 to 183 6±381 ng/mL p003 sEselectin 748± 154 vs 914± 122 ng/mL p00006 sPselectin 564± 137 vs 722± 149 ng/mL p0002 sCD40L 21± 09 vs 28± 11 ng/mL p003 and total 8isoPGF2alfa 2385± 470 vs 3078± 412 pg/L p00001 concentrations significantly increased Changes of circulating levels of sCD40L were directly correlated with changes of plasma total 8isoPGF2alfa r0523 p002 and sPselectin r0480 p003 levels
