Abstract
We present the case of a 55yearold woman who initially presented in 2005 to her local cardiologist with progredient dyspnoea fatigue and intermittent syncopal events Arterial hypertension moderate nicotine abuse adipositas and positive family history of coronary heart disease were preexisting cardiac risk factors Family history was negative for cardiomyopathic diseasesTwodimensional continuouswave Doppler echocardiography revealed a suspected maximal pressure gradient of 70 mmHg over the left ventricular outflow tract LVOT at rest why she was referred to our hospital for further evaluation of a suspected hypertrophic cardiomyopathy with obstruction HOCM and invasive diagnostic workup
