Abstract

Im Phasenmodell zur Prävention und Rehabilitation der DGUV Deutsche Gesetzliche Unfallversicherung wird bei psychischen Unfallfolgen psychologische Frühintervention gefordert Sie wird gleichermaßen als Stabilisierung frühe Behandlung und sekundäre Prävention verstanden Seit wenigen Jahren existieren wissenschaftlich fundierte psychologische Programme zur Frühintervention v a aus dem Bereich der kognitiven Verhaltenstherapie die oft unwirksame Debriefingansätze ablösten In der psychotraumatologischen Akutsprechstunde der Unfallbehandlungsstelle Berlin bewährte sich klinisch der Einsatz des Frühinterventionsverfahrens nach Bryant et al ergänzt um die spezifischen Erfordernisse des berufsgenossenschaftlichen Klientels Die Grundelemente Diagnostik bzw Risikoabschätzung Psychoedukation Affektkontrolle Expositionsvorgehen und kognitive Therapieanteile berücksichtigen in einem Zeitrahmen von 5 Therapiesitzungen das in der Traumatherapie übliche Phasenmodell Stabilisierung – Traumaexposition – Integration Eine wissenschaftlich fundierte Wirksamkeitsstudie zur Untersuchung des hier dargestellten Verfahrens steht noch ausThe phase model on prevention and rehabilitation of the German Statutory Accident Insurance stipulates that a psychological early intervention should be initiated for psychological sequelae of accidental injury Psychological early intervention consists of stabilization early treatment and secondary prevention Evidencebased psychological programs for early intervention have been available for a few years particularly in the field of cognitive behavioral therapy which replaced the often ineffectual debriefing approaches In the acute psychological consultation of the trauma emergency service in Berlin application of the early intervention method according to Bryant et al has proven successful supplemented by addressing the specific needs of the clientele of the employers’ liability insurance Over the course of five therapy sessions the basic elements of diagnostics as well as risk assessment psychoeducation affect control trauma exposure and elements of cognitive therapy incorporate the phase model customarily employed in trauma therapy consisting of stabilization – trauma exposure – integration A scientifically based efficacy study analyzing the procedure described here has not yet been conducted
