Abstract

Supine position hand supinated tourniquet loupe magnification Midlateral incisions along both sides of the finger running from the defect to the interphalangeal joint small defect or proceeding further proximally Careful elevation of the flap including both neurovascular bundles leaving dorsal branches of the bundles long fingers only and the flexor tendon sheath intact Suture of the flap in either flexion position ie advancement flap Moberg or by creating an islandflap through an additional transverse skin incision along the flap’s base O’Brien Finally closure of the defect at the flap’s base using a full thickness skin graft Z plasty or VY plastyRückenlage Hand seitlich ausgelagert Hand supiniert EsmarchBlutleere Lupenbrille Mediolaterale vom Defekt ausgehende Inzision an beiden Fingerseiten bis mindestens zum proximalen Interphalangealgelenk reichend bei kleinem Defekt Vorsichtiges Heben des Lappens inkl beider neurovaskulärer Bündel unter Schonung der dorsalen Äste nur Langfinger und der Beugesehnenscheide Einnähen des Lappens als Dehnungslappen in Flexionsstellung Moberg oder als Insellappen durch Anlage einer queren Hautinzision entlang der Lappenbasis O’Brien Bei Letzterem Verschluss des proximalen Defekts durch Vollhauttransplantation ZPlastik oder VYPlastik
