Abstract
I read with great interest the article by Vitale et al 1 on endoscopic retrograde cholangiopancreatography ERCP training It is clear that ERCP is the most difficult and complex procedure in digestive endoscopy Using this intervention the endoscopist has to deal with a great variety of situations In addition the ERCPrelated morbidity and mortality rates are higher than those for other endoscopic proceduresDeep cannulation into the desired duct seems to be one key that opens the gate to the vast field represented by pancreatobiliary endoscopy Furthermore it is well known that the bile duct the most frequent duct requiring cannulation is more difficult to cannulate than the pancreatic duct Therefore achievement of an acceptable success rate for bile duct cannulation ie 80 appears to be the primary goal in ERCP training 2 How many procedures a surgeon or gastroenterologist must perform to reach this level of capability is a matter of debate Gastroenterologists
