Abstract

The prevalence of spinal pathologies in subsaharan Africa has received little attention so far The aim of the survey was to investigate and compare the prevalence pattern of spinal lesions in two different populations of Tanzania one coming from the urban and semiurban catchment area of Aga Khan Hospital in Dar es Salaam and the other coming from the rural catchment area of Haydom Lutheran Hospital in Mbulu northern Tanzania At the Aga Khan Hospital a total number of 1163 patients were included in the survey In 50 cases 43 no radiological findings were available and 150 patients 129 showed no pathological abnormality 904 n = 1051 of screened population were classified in the category of extraduralextramedullary pathologies Intraduralextramedullary lesions were observed in only three cases 03 Intramedullary pathologies accounted for 21 n = 24 Mean age of the screened population was 466 years male–female ratio 1081 At the Haydom Lutheran Hospital 105 cases were included into the study Twentysix patients 248 showed no pathological abnormalities Extraduralextramedullary pathologies occurred in 724 n = 76 of overall cases However intraduralextramedullary pathologies were not seen in Haydom Intramedullary pathologies were diagnosed in two patients 19 Mean age was with 398 years clearly younger compared to urban areas male–female ratio being 1211 only nonsignificantly higher than in the Aga Khan Hospitals population Beside this one of the main goals of this study was to assess the frequency of infections to the spinal cord and vertebral column in rural and urban Tanzania Surprisingly there were only few radiological findings at the Aga Khan Hospital which suggested diagnoses consistent with tropical diseases a striking difference from rural Haydom Lutheran Hospital where nearly 30 of all patients showed changes on radiograph consistent with infections/infestationsBisher wurde der Prävalenz von spinalen Pathologien in afrikanischen Ländern südlich der Sahara nur wenig Aufmerksamkeit geschenkt Auf Grund dieser Tatsache wurde das Verteilungsmuster von spinalen Pathologien in zwei verschiedenen Populationen in Tansania untersucht einerseits im städtischen und halbstädtischen Einzugsgebiet des Aga Khan Hospitals in Dar es Salaam andererseits im ländlichen Einzugsgebiet des Haydom Lutheran Hospitals in Mbulu im Norden des Landes Insgesamt wurden im Aga Khan Hospital 1163 Patienten in die Studie eingeschlossen In 50 Fällen 43 waren keine radiologischen Endbefunde auffindbar 150 Patienten 129 präsentierten sich ohne pathologische Auffälligkeiten 904 n = 1051 aller untersuchten Patienten zeigten eine extraduralextramedulläre Veränderung in der Bildgebung Intraduralextramedulläre Pathologien wurden nur in drei Fällen 03 diagnostiziert Intramedulläre Veränderungen ergaben 21 24 Fälle aller Pathologien Das Durchschnittsalter der untersuchten Population war 466 Jahre Die Geschlechterverteilung männlichweiblich ergab 1081 Am Haydom Lutheran Hospital wurden 105 Patienten in die Studie eingeschlossen 26 Patienten 248 zeigten keine pathologischen Auffälligkeiten Extraduralextramedulläre Läsionen waren in 724 aller Fälle 76 Patienten zu sehen Intraduralextramedulläre Pathologien wurden nicht beobachtet Intramedulläre Läsionen wurden in 2 Fällen 19 diagnostiziert Das Durchschnittsalter war mit 398 Jahren deutlich jünger die männlichweiblich Ratio war mit 1211 geringgradig aber nicht signifikant höher Ein weiteres Ziel der Untersuchung war es die Häufigkeit von Infektionen des Rückenmarks und der knöchernen Wirbelsäule in Tansania zu erheben Überraschenderweise waren Infektionen im städtischen und halbstädtischen Einzugsgebiet des Aga Khan Hospitals kaum zu finden Im Gegensatz dazu fanden sich im ländlichen Bereich des Haydom Lutheran Hospitals in fast 30 aller Fälle Veränderungen in der Bildgebung die mit Infektionen oder Infestationen des Rückenmarks und/oder knöchernen Wirbelsäule einhergehen
