Effective penile block for circumcision in adults

Abstract

The dorsal nerve block of the penis during circumcision has not been standardized and the effectiveness of the block can be changeable The aim of this study was to determine which technique is more suitable for blocking sensory innervations of penisThis prospective study was done on adults who voluntarily applied to be circumcised Patients were randomized into three distinctive groups first just circular infiltration second circular plus dorsal penile infiltration and third circular plus ventral upward infiltration Sensation was tested using pinprick sensory testing before and after infiltration The total duration of the procedure was recordedIn total 89 adult uncircumcised men were included into the study The average age of men was 214 min 20 max 25 The average operation time in the third group was significantly shorter The sensation distribution especially around the frenulum sensitivity results were in the first minute 100 813 724  in the third minute 857 375 207  and in the fifth minute 571 188 and 0  for the first second and third group respectively In the fifth minute there was no sensitivity for any patients in the third group and it was significantDie dorsale Nervenblockade des Penis während Zirkumzision ist nicht standardisiert Ihre Wirksamkeit kann wechselhaft sein Ziel dieser Studie war es herauszufinden welche Methode am geeignetsten zur Blockade der sensiblen Innervation des Penis istDiese prospektive Studie wurde an Erwachsenen durchgeführt die sich freiwillig zur Zirkumzision meldeten Die Patienten wurden randomisiert 3 Gruppen zugeteilt Gruppe 1 nur zirkuläre Infiltration Gruppe 2 zirkuläre Infiltration plus Infiltration des Dorsum Penis Gruppe 3 zirkuläre Infiltration plus ventral aufsteigende Infiltration Mittels Nadelstich Testung wurde die Empfindlichkeit vor und nach der Infiltration geprüft Die Gesamtdauer der Prozeduren wurde festgehaltenEs wurden insgesamt 89 nicht beschnittene Männer in die Studie aufgenommen Das Durchschnittsalter betrug 214 Jahre min 20 max 25 Im Durchschnitt war die Gesamtdauer der bei Gruppe 3 angewandten Prozedur signifikant kürzer Die Verteilung der Empfindlichkeit besonders um das Frenulum herum war entsprechend der Sensititvitätstestung in den einzelnen Gruppen unterschiedlich in der 1 Minute 100  bei Gruppe 1 813  bei Gruppe 2 724  bei Gruppe 3 in der 3 Minute 857 375 207  und in der 5 Minute 571 188 and 0  In der 5 Minute war bei allen Patienten der Gruppe 3 eine Unempfindlichkeit erreicht Damit unterschied sich die Gruppe 3 signifikant von den anderen Gruppen
