Abstract
Cyclosporine has been shown to be effective in treating steroidrefractory ulcerative colitis UC1 It has FDA pregnancy categoryC status and the American Academy of Pediatrics recommends mothers avoid breastfeeding while on cyclosporine2 Herein we report both maternal and neonatal levels of serum cyclosporine in a woman with UC treated with oral cyclosporine during pregnancy and while breastfeeding in the postpartum periodA 36yearold woman was admitted at 26 weeks of pregnancy with a severe flare of her leftsided UC She did not respond to intravenous IV steroids for 5 days so IV cyclosporine was used with good effect After 5 days the IV cyclosporine was changed to oral 5 mg/kg/day and she was discharged home She was unable to come off oral cyclosporine for the remainder of the pregnancy with trough serum cyclosporine levels between
